Insurance for Medical Visits:
We are in-network with Anthem / Blue Shield PPO, Blue Cross PPO, UnitedHealthcare PPO, Healthnet PPO, and Medicare plans. We also welcome self-paying patients - we can provide you with a superbill to submit to your insurance for out-of-network reimbursement if your plan allows.

Cosmetic Consultation Policy:
Please note that a $250 consultation fee is collected at the time of your visit for both new and established patients, which will be fully applied toward the treatment of your choice. If you already know you want treatment same-day, please mention that when requesting an appointment.

Cancellation & No-Show Policy:
We require 48 hours’ notice. Cancellations within 48 hours incur a $150 fee charged to the card collected at the time of booking.
